Savings Estimate, Preventive Care Management Solution Proposal

SIMERPU recap and commentary: Mardy Gould

Opened — main text retrieved

Quick recap

Evaluates a municipal PCMP proposal. Finds that the modeled service fee exceeds Medicare-only savings for much of the workforce; also discusses assumptions and operational considerations.

SIMERPU’s viewpoint

Chicago’s analysis demonstrates why workforce tax participation can materially change a savings illustration. SIMERPU would treat employer economics as a separate question from service value and the legal analysis of the structure. The report’s Medicare-only example is particularly useful for public employers evaluating a proposal.

What this source contributes

The analysis tests fees against the taxes actually applicable to a municipal workforce instead of assuming one savings rate fits everyone.

What remains unanswered

Its findings are specific to the modeled proposal and workforce. They do not measure every program’s service value or settle its tax treatment.

Mardy Gould’s take

This is exactly why I want the employer’s actual payroll facts in the conversation. A proposal can offer services worth considering and still have economics that differ from a standard example. I would separate those two discussions, show the net numbers and let the employer evaluate the complete proposition.

Questions I would bring to the conversation

1. Which payroll taxes apply to the participating employees?

2. Do fees exceed modeled savings for any employee group?

3. How is service value evaluated separately from tax savings?

What would change our assessment?

Different workforce tax participation, fees or program terms could change the financial result and should be modeled explicitly.
